Buzzair and Natech - yeah, when FrecklesFly and I were in Europe, we saw several people sign Cocklear Implant with four fingers, it is not only a German usage, but pretty much widespread in Europe.

I like the German (I'm assuming that's German) sign for CI. Seems more invasive in their language.
